Medication

 

Individuals must have a current prescription, meet the income guidelines (outlined below).  If you do not have a physician, please review our list here – Need a Doctor.

Qualification Requirements

Must be uninsured / underinsured (with household budget info) and a Stark or Carroll County resident  Must be at or below 250% of Federal Poverty Guidelines:

 Must bring for an appointment

Income Verification for all household members  *At least 2 paystubs for each member & Prior year tax return if filed
Photo Identification & Verification of address if not the same as what is on the Photo ID
Social Security Card
Bring Prescriptions, medicine bottles, (must have current scripts or refills available.)

Note: Beacon Charitable Pharmacy is a charitable pharmacy that receives donated medications. Therefore, Beacon cannot guarantee what medications are available. Beacon does not have any controlled substances. Beacon has several other resources such as Medication Assistance Programs (MAPs) through the pharmaceutical companies to help individuals obtain medications not available in the charitable pharmacy.
